August 28, 2025 - Council authorized negotiation and execution of an amendment to the agreement with Get Up Project d/b/a Hope Medical Clinic to provide health education and navigation services, to add one 12-month extension option for the term ending September 30, 2026, in an amount not to exceed $150,000, for a revised total agreement amount not to exceed $500,000. Approved on a 10-0 vote.
September 12, 2024 - Council authorized negotiation and execution of an amendment to the agreement with Get Up Project d/b/a Hope Medical Clinic to provide health education and navigation services, to add one 12-month extension option for the period ending on September 30, 2025, and add funding in an amount not to exceed $150,000, for a total agreement amount not to exceed $350,000, approved on an 11-0 vote.

This action will authorize negotiation and execution of Amendment No. 6 to the agreement with Get Up Project d/b/a Hope Medical Clinic to provide health education and navigation services prioritizing the Asian American and Pacific Islander communities. Get Up Project provides case management, chronic illness support groups, education classes, translation services, and referral services for physical, dental, behavioral, and mental health needs. It has a successful track record serving the Asian American and Pacific Islander communities.
The agreement was renewed from 2023 to 2026, including amendment No. 1 through No. 5.
