Approval of this item will authorize Amendment No. 4 to the Permanent Supportive Housing North Program to add two 12-month extension options beginning on October 1, 2025. The program provides intensive housing and community-based treatment services for individuals experiencing homelessness with complex needs to obtain and maintain housing stability. The program objectives promote wellness by assisting individuals maintain housing, avoiding hospital inpatient readmissions, avoiding involvement in the criminal justice system, promoting medication adherence, and recovering from substance use. The program provides outreach and engagement, case management, psychosocial rehabilitation, referrals to vocational and employment services, psychiatric services, primary care, and connection to other services as needed by the individuals served through an onsite-based program staff. If authorization of Amendment No. 4 is not approved, it will limit the Homeless Strategy Office's ability to provide permanent supportive housing services.
