Approval of this item will authorize the Homeless Strategy Office (HSO) to negotiate and execute an agreement with Foundation Communities beginning on September 1, 2025, to provide supportive services that help families experiencing homelessness find appropriate housing to end their homelessness. The agreement may also include short-term services to connect families to benefits and resources that promote long-term stability and reduce the risk of future homelessness. Additionally, the agreement aims to improve housing outcomes by strengthening coordination between service providers, including organizations that primarily serve families experiencing homelessness and those that work with very low-income families, such as health care providers, schools, workforce development agencies, youth programs, and counseling services.
This agreement was awarded through the HSO's Request for Applications (RFA): 2025-002 Supportive Services for Families Experiencing Homelessness. HSO solicited more than 650 vendors, and HSO received eight applications. Foundation Communities was recommended for an award with a 24-month term, without the option to renew.
