Approval of this item will authorize Amendment No. 4 to the Rehousing Aging Adults Program agreement to add funding in the amount of $882,435 beginning on February 1, 2025. The program helps clients transition from homelessness into permanent housing and provides supportive services to their housing stability. Supportive services include peer support, housing search assistance, financial assistance, and case management. Throughout their contract, Family Eldercare has served 192 unduplicated clients. If authorization of Amendment No. 4 is not approved, then it will limit the Homeless Strategy Office's ability to provide rapid rehousing services.
