This action will authorize the negotiation and execution of Amendment No. 12 to the agreement with the Asian American Resource Center d/b/a Austin Asian Community Health Initiative, for the Community Health Navigator program to provide community health system navigation services for members of linguistically isolated Asian American immigrant groups.
Amendment No. 11 added a 12-month extension option and exercised the eighth extension option in the amount of $587,305 for the period of October1, 2023, to September 30, 2024.
Amendment No. 10 exercised the seventh extension option in the amount of $587,305 for the period of October 1, 2022, to September 30, 2023.
Amendment No. 9 added a 12-month extension option for $587,305, exercised the sixth extension option, and added $350,000 in funding for the period of October 1, 2020, to September 30, 2022.
Amendment No. 8 added two 12-month extension options beginning on October 1, 2020, in an amount not to exceed $412,305 for each extension option and exercised the fifth extension option.
Amendment No. 7 exercised the fourth 12-month extension option in the amount of $412,305 for the period of October 1, 2019, to September 30, 2020.
Amendment No. 6 exercised a legal name change to the Asian American Community Health Initiative.
