Authorize negotiation and execution of an amendment to an agreement through September 30, 2025, with Youth and Family Alliance d/b/a LifeWorks to provide rapid rehousing services for youth and young adults experiencing homelessness, to add one-time funds in an amount not to exceed $1,187,565 for a revised total agreement amount not to exceed $4,234,480. This action will authorize Amendment No. 4 to add one-time funds in the amount of $1,187,565 beginning September 1, 2025, to the Homeless Strategy Office's Rapid Rehousing Youth agreement with LifeWorks. The program is designed to rapidly house youth ages 18-24 in scattered-site rapid rehousing apartments and provide them with comprehensive supportive services. Throughout this contract, LifeWorks has served 177 unduplicated clients.
