Approval of this item will authorize negotiation and execution of an agreement with Get Up Project d/b/a Hope Medical Clinic to provide health education and navigation services prioritizing the Asian American and Pacific Islander communities.

 

Get Up Project provides case management, chronic illness support groups, education classes, translation services, and referral services for physical, dental, behavioral, and mental health needs.  It has a successful track record serving the Asian American and Pacific Islander communities.  The Get Up Project submitted all required documentation and passed the minimum qualifications to receive social services funding.
